No. NExT regulations were notified by the NMC in 2023, but implementation has been put on hold indefinitely after Ministry of Health and Family Welfare intervention following protests from the 2019 MBBS batch. The Foreign Medical Graduate Examination (FMGE) remains the qualifying exam for foreign medical graduates until NExT is rolled out. Q: How often must a medical college renew its permission to admit MBBS students?

Annually. Under Regulation 4 of the Maintenance of Standards of Medical Education Regulations, 2023 (MSMER 2023), every college holding a valid Letter of Permission (LOP) must submit an Annual Disclosure Report on the NMC portal each year as a mandatory prerequisite for seat renewal, with a hard deadline and no extensions. For Academic Year 2026-27 the notified fee was INR 3,54,000 inclusive of 18 percent Goods and Services Tax (GST), a figure tied to that specific cycle rather than a fixed standing cost.

Q: Did the NMC increase or decrease infrastructure requirements for new medical colleges under MSR 2023?

Both, depending on the metric. Bed, faculty and outpatient-load requirements were sharply increased under Minimum Standard Requirements (MSR) 2023, while a fixed minimum land-area mandate that existed under the earlier framework was removed. This is a genuine mixed picture, not a uniform loosening or tightening, and colleges should verify current thresholds directly against the notified MSR 2023 text before relying on any specific figure.

Why Most Medical Colleges Are Only Half-Compliant

Medical college promoters and managements in India put enormous effort into establishment-phase compliance: Medical Assessment and Rating Board (MARB) clearance, Minimum Standard Requirements (MSR) infrastructure, and faculty recruitment. These are necessary, but they cover only part of a regulatory landscape that keeps moving after the college is approved.

National Medical Commission (NMC) rules are amended frequently, and several of the regulations that govern a medical college, including MSR, the Maintenance of Standards of Medical Education Regulations (MSMER) 2023 annual renewal cycle, and the National Exit Test (NExT), were revised or introduced as recently as 2023 to 2026. A compliance position built on a single snapshot, an outdated set of MSR figures, or an assumption that NExT already applies, is only half right.
